Omaha Animal Medical Group offers our patient form(s) online so you can complete it in the convenience of your own home or office.

• Download the necessary form(s), print it out and fill in the required information.
• Fax us your printed and completed form(s) to (402) 496-1619 or bring it with you to your appointment.

Care Credit is a separate line of credit, which does not affect the balance of your other credit cards.

Unlike other credit cards, there are no annual fees.

There is no interest applied if the amount financed is $200 or more, and payments are based on the total amount financed, divided into 6 months.

This monthly payment plan does not require a payment now, and processing your application will only take a few minutes. You can fill out an application in our office or click the Care Credit button to apply online.

Preferred Client Dental Program

Our Preferred Client Dental Program allows you to make payments to be put towards your pet’s dental cleaning. Once a portion of the total cost has been reached, we will schedule the cleaning. Afterwards, your payments continue until your account is paid in full.
